Modernised Care Pathway for patients with Inflammatory Bowel Disease

A modernised Care Pathway for patients with Inflammatory Bowel Disease (IBD) was operational across 11 sites during the summer.

It was actively improving care for IBD patients by streamlining referrals through nurse-led triage, expanding the role of IBD Clinical Nurse Specialists and Advanced Nurse Practitioners and  increasing access to specialist care by supporting patients in the community, not just hospitals.

This approach improved outcomes, reduced pressure on hospitals and supported Sláintecare’s goal of integrated care.

Over 50,000 people in Ireland live with Inflammatory Bowel Disease (IBD), yet many have had to face a long wait for diagnosis, often presenting to ED for symptom management.
